How to get the service online:
- Log in on the portal and click the Request Online button.
- Fill in all required fields.
- Sign it by EDS (electronic digital signature) or by SMS-password (be sure to have registration in the mobile citizens database).
- In personal cabinet (in the History of Services section) read the generated electronic document and notification of the request result.

- Identity document for identifying the person when directly contacting the service provider.
- Application in the form according to Appendix 1, 2.
