How to get the service at State agency (required documents):
For registration of paternity establishment by the parents' joint application:
- Application.
- Birth certificate, in case of application submission before its birth - medical certificate to confirm the pregnancy of mother issued by the medical organization or private practitioner. If state registration of the paternity establishment is conducted simultaneously with state registration of the child birth birth certificate is not required;
- Copy of ID card.
For the registration of the paternity establishment upon the application of the father of a child in case of mother's death, announcement of mother's death, disability of mother because of the mental diseases, termination of parental rights, impossibility of the mother's place of residence detection:
- Application.
- Documents which prove the circumstances of the mother's absence: death certificate, court decision on incapability of mother because of mental disorder, declaration of death, termination of parental rights, certificate on the impossibility to detect the place of residence of mother.
- Copy of ID card.
For the registration of the paternity establishment by the court decision:
- Application.
- Copy of the court decision paternity establishment.
- Certificate of the child birth.
- In case of one of the parent’s death – death certificate.
- Copy of ID card.
For amend, changes and additions in vital acts record:
- Application.
- Court decision copy.
- Birth certificate (if the original certificate is lost, indicate this in the application).
- Certificate of paternity (if the original certificate is lost, indicate this in the application).
- Documents which confirm the necessity of making amend and changes and additions.
- Document confirming the payment of state fee to budget or document which confirm the tax allowances.
- Document which prove the authority of representative.
- ID card of the parents.
