Assignment of monthly payments to citizens of the Republic of Kazakhstan after capitalization period on compensation of harm to life and health of employees, inflicted by legal entities liquidated because of bankruptcy

How to get the service  in "Government for citizens" State Corporation NJSC (required documents):

1. Application
2. Identity document. 
3. Information on bank account number in organization issuing monthly payments or special account of  correctional facility.  
4. Copy of court order, confirming the right to receive capitalized amounts, containing information on capitalization period, amount of capitalized payments on compensation for harm:

• upon receipt of the amounts of capitalized payments from the state – entered into force decision (resolution, definition) on the imposition of liability for the payment of capitalized payments for damages to the state.
• upon receipt of the amounts of capitalized payments at the expense of the property of the liquidated legal entity – determination (decision) on completion of bankruptcy proceedings with the approval of the final report of the bankruptcy Trustee, the materials of which contain a transcript for each creditor of the first stage.
• The right to receive capitalized amounts can also be confirmed by an archival document of the court or the state archive containing information about the capitalization period, the amount of capitalized payments for compensation for damage.

5.  Depending on the status of the victim:

• at compensation of the harm caused by damage of health – the copy of the reference on establishment of degree of loss of professional working capacity.
• in case of compensation for damage to persons with disabilities who suffered damage as a result of the death of an employee, who in accordance with paragraph 3 of article 940 of the Civil code of the Republic of Kazakhstan shall be compensated for the period of disability – a copy of the certificate of disability.

In case of availability of data on establishment of degree of loss of professional working capacity or disability in the judicial act or the archival document confirming the right to the capitalized amounts, submission of the separate reference is not required.
