Registration of death which includes making amendments, addition and corrections in the vital record
Dear citizens of the Republic of Kazakhstan!
In order to prevent the spread of coronavirus infection in the Republic of Kazakhstan, the provision of services on paper media is possible through online reservations in the service centers of the population for the period of quarantine.
Public services can be obtained online, as well as some services are available in the mobile app EgovMbile, Telegram-bot eGovKzBot2 .0 and social networks Facebook and VK.
How to get the service online:
- Log in on the portal and click the Request Online button.
- Fill out the application and sign it by EDS (electronic digital signature) or by SMS-password (be sure to have registration in the mobile citizens database).
- In personal account (in the History of services section) read the notice of processing your application, which will be received within the specified time.
How to get the service at State agency and at «State-owned corporation «Government for citizens» NJSC (required documents):
- Application for state registration of death.
- Information about a death certificate issued by a medical organization.
- ID card of the deceased (if available, in the absence of the identity of the deceased, the reasons must be indicated in the application).
- Notary certified letter of attorney in case of service recipient’s representative addressing.
- In case of registration of death on the basis of a court decision-information about a court decision that has entered into force to establish the fact of death or to declare a person dead.
- Final judgement on establishment of death fact or on announcement of person's death, in case of registration on the basis of court decision.
- Military ID of the deceased (if available)
- If necessary (if the surname and / or patronymic of the service recipient is not related), a document confirming close relationship.
List of required documents for changes, additions and corrections entering to the death record:
- Application for making changes, additions and corrections.
- ID card (for identification purposes).
- The original certificate of state registration of civil status, which is to be exchanged in connection with the amendments, additions and corrections to the record of civil status, in case of loss of the original certificate to indicate that it is lost and information about the registration of a civil status act.
- Document confirming the existence of grounds for making changes, additions and corrections to the civil status record.
- Notary certified letter of attorney in case of service recipient’s representative addressing.
Result of the service delivery
Certificate or reference of death, duplicative certificate of death with entered changes, additions and corrections or a motivated answer about refusal to provide a public service or a motivated answer about refusal to provide a public service.
The form of providing the result of public service provision: paper, electronic when a certificate of death is sent, a reasoned refusal when submitting an application through the portal.
