How to get the service in the «State-owned corporation «Government for citizens» NJSC (required documents):
- Statement.
- An identity document or an electronic document from the digital documents service (required for identity identification).
- Notarized consent of the spouse(s), if married.
- Certificates on the health status of the service recipient and the spouse(s), if married, confirming the absence of diseases in accordance with the list, as well as certificates on the absence of information on the condition registered in narcological and psychiatric dispensaries in accordance with the form approved by the order;
- A copy of the marriage certificate (if married) in the absence of information in the information system "Registry Office" (hereinafter referred to as the Registry Office) or outside the Republic of Kazakhstan.
- A copy of the birth certificate of a child (children), in the absence of information in the is registry office or born outside the Republic of Kazakhstan (the original is required for identification).
- Copies of the documents confirming the fact of lack of custody of the child only, or both parents (a death certificate, court decision on deprivation of parental rights, restriction of their parental rights, recognition of parents is unknown absent, incapacitated (ogranichenno capable), declaring them dead, the verdict on serving parents of punishment in places of deprivation of liberty, proof wanted by the parents, the child (children) of the parents, presence of parents on long-term care in health care organizations, act of giving up a child( children), statement of rejection of the child (children), birth certificate (in the absence of information in the civil registry office) in the form.
- Information about the income of the service recipient and (or) spouse(s), if married.
- Copies of documents confirming the fact of kinship of relatives, stepfather (stepmother) to the child (children).
- Certificate of completion of training for persons wishing to adopt orphaned children and children left without parental care (with the exception of close relatives of the child).
- Copies of documents confirming the right to use the housing of the service recipient and (or) spouse (s) (in the absence of ownership of housing).
The documents are submitted in originals for verification, after which the originals are returned to the service recipient.
When referencing the portal:
- an application in the form of an electronic document signed by the EDS of the service recipient or certified with a one-time password, in the case of registration and connection of the subscriber number of the service recipient provided by the mobile operator to the portal account;
- an electronic copy of the notarized consent of the spouse (s), if married;
- an electronic copy of the health certificate of the service recipient and the spouse (s), if married, confirming the absence of diseases in accordance with the list approved by order № 692, as well as a certificate of the absence of information about the state registered in drug treatment and psychiatric dispensaries in accordance with the form approved by order № 907;
- electronic copy of the marriage certificate, if married, in the absence of information in the information system of the civil registry office or outside the Republic of Kazakhstan;
- electronic copy of the birth certificate of a child (children), in the absence of information in the information system of the civil registry office or born outside the Republic of Kazakhstan (the original is required for identification);
- electronic copies of documents on the income of the service recipient and (or) the spouse (s), if married;
- electronic copies of the documents confirming the fact of lack of custody of the child only, or both parents (a death certificate, court decision on deprivation of parental rights, restriction of their parental rights, recognition of parents is unknown absent, incapacitated (ogranichenno capable), declaring them dead, the verdict on serving parents of punishment in places of deprivation of liberty, proof wanted by the parents, the child (children) of the parents, presence of parents on long-term care in health care organizations, act of throwing up a child( children), application for refusal of the child( children), birth certificate (in the absence of information in the civil registry office) in the form approved by order № 112 (registered in the Register of state registration of normative legal acts of the Republic of Kazakhstan under № 10764);
- electronic copies of documents confirming the right to use the housing of the service recipient and (or) the spouse (s) (in the absence of ownership of the housing);
- an electronic copy of the opinion of the child (children) (when reaching the age of ten years) in accordance with Annex 12 to the Rules for the provision of public services in the field of family and children, approved by this order;
- an electronic copy of the certificate of completion of training for persons who want to adopt orphaned children and children left without parental care.
Provision of the birth certificate of the child (children) and the documents specified in sub-item 7) of the list provided to the service provider and to the State Corporation is not required if the child (children) lives in organizations for orphans and children left without parental care.
If necessary, the service recipient can apply for the state service " Purpose of payment of benefits to guardians or guardians for the maintenance of an orphan child (s) and a child (s) left without parental care", which will be provided after the establishment of guardianship or guardianship over an orphan child (children) and a child (children) left without parental care.

Reception is carried out in the order of "electronic" queue, at the place of registration of the service recipient, or at the place of registration of a minor in need of guardianship.
