How to get online service
- Log in on the portal and click the Request Online button.
- Fill out the application and sign it by EDS (electronic digital signature) or by SMS-password (be sure to have registration in the mobile citizens database).
- Make a payment by clicking the "Pay" button on the page of the Payment Gateway of the "e-government".
- In personal account (in the History of Services section), read the notice of processing your application, which will be received within the specified time.
How to get the service in NJSC State Corporation "Government for Citizens" (necessary documents)
Making changes, additions and corrections to the act record on the conclusion or dissolution of marriage (matrimony)
- Application for making changes, additions and corrections.
- An identity document or an electronic document from the digital documents service (for identification).
- The original certificate of state registration of civil status, which is to be exchanged in connection with the amendments, additions and corrections to the record of civil status, in case of loss of the original certificate to indicate that it is lost and information about the registration of a civil status act.
- Document confirming the existence of grounds for making changes, additions and corrections to the civil status record.
- Notary certified procuration in case of service recipient’s representative addressing.
Making changes, additions and corrections to the record of the birth certificate.
How to get the service in NJSC State Corporation "Government for Citizens" (necessary documents)
- Application for making changes, additions and corrections.
- An identity document or an electronic document from the digital documents service (for identification purposes).
- Birth certificate, in case of loss, indicate its loss and information about the state registration of the act of civil status.
- Copies of certificates of state registration of civil status acts issued outside the Republic of Kazakhstan, legalized or stamped with an apostille, with a notarized translation into Russian or the state language, confirming the need to make changes, additions and corrections to the birth certificate.
- Information on documents confirming the need for amendments, additions and corrections (information on civil status acts registered in the territory of the Republic of Kazakhstan, information on the court decision on amendments, corrections, additions, on the establishment of a legal fact, and so on).
- A document confirming the payment of a state fee to the budget or a document that is the basis for granting tax benefits (if necessary, its payment).
- A notarized power of attorney, in the case of a request from a representative of the service recipient.
The list of necessary documents for making changes, additions and corrections to the birth certificate when contacting the service provider
- Application for establishing paternity (motherhood), for establishing paternity at the request of a person who recognizes himself as the father of a child, for establishing paternity by court decision, for adoption of a child, for making changes, additions and corrections in the form according to Appendix 8, 9, 10, 12, 24 to the Rules (depending on the basis of making changes, additions and corrections).
- ID card (for identification purposes).
- Birth certificate, in case of loss, indicate its loss and information about the state registration of the civil status act.
- Information about documents confirming the need to make changes, additions and corrections (information about the court decision on adoption, establishment of paternity; about the court decision to cancel or recognize the adoption as invalid; when establishing paternity, documents confirming the circumstances of the absence of the mother, depending on the grounds specified in the application of the father: information about the registration of the civil status act on the death of the mother (on the territory of the Republic of Kazakhstan); information about the court's decision to recognize the mother as incapacitated, to declare her dead, to deprive or restrict the mother of parental rights; a certificate of inability to establish the mother's place of residence, information about the court's decision to make changes, corrections, additions, to establish a legal fact, etc.).
- Copies of certificates of state registration of civil status issued outside the Republic of Kazakhstan, legalized or stamped with apostille, with a notarized translation into Russian or the state language, confirming the need to make changes, additions and corrections to the birth certificate.
- Document confirming payment of the state fee to the budget or a document that is the basis for granting tax benefits (if necessary, payment of it).
- Notary certified procuration in case of service recipient’s representative’s addressing.
- If an application is submitted to establish paternity before the birth of the child, a medical certificate confirming the mother's pregnancy issued by a medical organization or private practitioner.
- If paternity is established simultaneously with the state registration of birth, the child's birth certificate is not required.
